Alarms ignore Silent Mode, Focus and Do Not Disturb by default. Notifications do not. So the fix is never a louder notification, it is an actual alarm.
People search for this expecting a hidden switch. There is no switch, because the behaviour is already what you want: iOS lets alarms through everything. The reason your phone stayed quiet is that whatever tried to reach you was not an alarm.
What does iOS let through a silenced phone?
Three things. Alarms and timers from any app that is allowed to schedule them, calls from contacts you have marked as allowed, and Emergency Alerts. How do you make sure the alarm is loud enough?
Alarm volume on iPhone is separate from media volume and is not changed by the side buttons while music is playing. Check Settings → Sounds & Haptics → Ringtone and Alert Volume, and turn off Change with Buttons if you keep knocking it down by accident. The background is in changing the calendar alert sound.
A volume ramp helps more than raw loudness. Starting quiet and reaching full volume over about twenty seconds wakes you without the jolt, and it still gets there if you sleep through the first few seconds.
How do you get an alarm for a calendar event?
